Hi Marla…..I have experienced similar with larger institutions/hospitals. The reason they site is because they have too many studies/sponsors/IRTs and cannot accommodate each Sponsor’s differences. But the usually use their own internal forms and provide those to the Monitor and Sponsor. Then the monitor can complete the destruction in the IRT.
